Which Lasting Power of Attorney forms do I need?

Forms for LPA

This page explains which Lasting Power of Attorney forms you need to make each type of Lasting Power of Attorney.

There are three ways to get the forms required to make a Lasting Power of Attorney. You can:

  • Fill in the forms online then print out the forms and sign them
  • Download the blank forms, print them off and complete them or
  • Visit a solicitor or lawyer who will complete the forms for you.

You will see that you have the option of downloading either:

  • A complete info pack for the finance/property or health/welfare power of attorney, or
  • The individual forms.

Large print versions of the forms are also available.

The Health and Care Lasting Power of Attorney complete pack includes:

  • Form LP1H – the form you need to create and register your LPA.
  • LP3 – the form you can use to notify people that you have made an LPA.
  • LP12 – a guide to making and registering your LPA
  • LPA120 – the fees for making a health and care LPA together with the application form you need if you want to apply for fee remission
  • LPC continuation sheets – used if you want to:
    • Continuation sheet 1 – name additional people in sections 2, 4 or 6 of the LPA form
    • Continuation sheet 2 – provide additional information in sections 3, 4 or 7 of the LPA form
    • Continuation sheet 3 – fill in to show that the donor cannot sign or make a mark themselves
    • Continuation sheet 4 – use where a trust corporation is appointed as an attorney

Note that there is a specific order for signing all documents.

  • Form LP1f – the form you need to create and register your LPA

  • LPA120 – the fees for making a finance and property LPA together with the application form you need if you want to apply for fee remission
  • LPC continuation sheets – used if you want to:

    • Continuation sheet 2 – provide additional information in sections 3, 4 or 7 of the LPA form

Note that there is a specific order for signing all documents. If you do not sign the forms in the correct order, your application will be refused. From 2012 to 2015 the Office of the Public Guardian received over a million applications to register a Lasting Power of Attorney. Of those, some 130,000 contained mistakes that prevented them from being registered. We strongly recommend you use a lawyer to help you complete the Lasting Power of Attorney forms. Using an experienced lawyer who specialises in this area of law helps ensure that those mistakes are avoided.
